Account notes not retained after creating account

I do not see a "modify account" link.

If you click on 'Accounts', and one of your username links (phone numbers), the account profile for that account shows up. You can then modify things, such as notes, international calling, access template, etc., then click 'Modify Account' to file your changes.

Modifications to an account via this method appear to be retained in the database properly. However, when creating accounts for the first time, and populating the 'Notes' field, the 'Notes' information is not retained.
