As an observation, you don't seem to be doing a real LIDB dip for my cell phone.
At http://www.cnam.info/ it gave my real name for my number.

(It was likely set while at Packet8.net)

 When I call my VOIPo number it says "Livonia MI" instead of <my name>.

I wonder if that is because the LIDB you are using comes up empty,
and VOIPo defaults to an NPA cross reference?
Or if something else makes my name unavailable
